5. Keep your details of Debit Card/Credit Card/Net banking ready for online payment of Rs. 25/- as the registration amount.
6. In the case of the single parent, the phone no. of the parent is to be repeated in both the columns.
7. Fill up the form completely before taking a printout. In case of incomplete form/documents, the application will be deemed as rejected.
8. Print out of the filled registration form, duly signed by the parent, must be submitted at the fee counter, along with self-attested photocopies of the supporting documents.
9. Non-submission of the filled registration form along with self-attested supporting documents will make you non-eligible for the draw for the final list.
10. The last date for submission is 27.12.2019 up to 02:00 PM.
